What is “Do Not Disturb while driving”?
When “Do Not Disturb while driving” is active, it helps you stay focused on the road:

Text messages are silenced and don’t appear onscreen. By default, iPhone sends an auto-reply, which you can customize. When someone responds to your auto-reply with the message “Urgent,” all subsequent texts from that person come through for the remainder of your drive.
Notifications are silenced, except for emergency alerts, timers, and alarms.
Phone calls are allowed only when your iPhone is connected to your car’s Bluetooth system or a hands-free accessory, or you use the Do Not Disturb settings to allow some calls.
Siri reads replies aloud, so you don’t have to look at your iPhone (the screen stays dark).

Turn on “Do Not Disturb while driving”
If iPhone detects you might have driven a vehicle before you set up “Do Not Disturb while driving,” iPhone asks if you want to turn it on. Otherwise, to set it up, follow these instructions:
Go to Settings > Do Not Disturb > Activate, then choose an option:

Automatically: “Do Not Disturb while driving” activates when iPhone senses you might be driving. If it becomes active when you’re not driving (if you’re a passenger in a vehicle, for example), tap the Do Not Disturb notification on the Lock screen, then tap I’m Not Driving.
Manually: You activate “Do Not Disturb while driving” from Control Center. To add the option to Control Center, Go to Settings > Control Center > Customize Controls, then tap + next to Do Not Disturb While Driving.
When Connected to Car Bluetooth: This option activates “Do Not Disturb while driving” when your iPhone is paired using Bluetooth.

If your car supports CarPlay and your iPhone is connected, Do Not Disturb while driving isn’t activated. Instead, CarPlay uses your car’s built-in display to help you stay focused on the road.
Allow some calls
If your car doesn’t have Bluetooth, you can allow some calls to go through.

Allow a second call from the same person within 3 minutes: Go to Settings > Do Not Disturb and turn on Repeated Calls.
Allow calls from your Favorites or everyone: Go to Settings > Do Not Disturb > Allow Calls From.

Create a custom auto-reply text message
To change the auto-reply message (“I’m driving with Do Not Disturb While Driving turned on. I’ll see your message when I get where I’m going”), go to Settings > Do Not Disturb > Auto-Reply, then tap the message.
By default, your auto-reply is sent to your Favorites. To change who receives the auto-reply or to turn it off, go to Settings > Do Not Disturb > Auto-Reply To, then select a group:

Recents: Sends an auto-reply to anyone you sent a message to in the previous two days, even if they’re not in your Contacts.
All Contacts: Sends a message to any of your Contacts.
No One: Turns off auto-reply.

About Do Not Disturb and privacy
“Do Not Disturb while driving” uses Location Services to help determine if you might be driving or if you’re near your home, work, or a predicted destination. The location data Apple collects for these purposes doesn’t personally identify you. To turn off Location Services for “Do Not Disturb while driving,” go to Settings > Privacy > Location Services > System Services, then turn off Location-Based Alerts.
